Proopiomelanocortin (POMC) is a precursor protein that cells cut into smaller peptides, including ACTH, the melanocyte-stimulating hormones (MSH) and beta-endorphin. Published work links POMC-derived peptides to the stress axis, pigmentation, pain signalling and appetite. Proopiomelanocortin (POMC) is a prohormone — a single long polypeptide that enzymes inside secretory cells cleave into several smaller, biologically active peptides. Those products include adrenocorticotropic hormone (ACTH), the melanocyte-stimulating hormones (α-, β- and γ-MSH), β-lipotropin and the opioid peptide β-endorphin. Because one gene yields many signalling molecules, POMC is one of the most frequently cited examples of post-translational processing in endocrinology. A 2019 review of POMC evolution described the gene as ancient and broadly conserved across vertebrates, with tissue-specific processing enzymes generating different peptide repertoires in different cell types (PMID 31421696).

Where POMC Is Produced

POMC expression is not confined to one organ. The classic sites are the anterior pituitary corticotrophs, which release ACTH into the circulation, and the intermediate-lobe melanotrophs of many species. A second major site is the hypothalamic arcuate nucleus: a 2013 review of central POMC neural circuits described arcuate POMC neurons and their projections as a core component of the melanocortin system involved in energy homeostasis (PMID 23440036).

POMC is also expressed peripherally. A 2023 review in the psychodermatology literature described a cutaneous POMC system in keratinocytes, melanocytes and immune cells, and discussed POMC-derived peptides as candidate mediators linking stress signalling with skin biology (PMID 37275429).

What Studies Report About POMC Function

Feeding and appetite

Much of the experimental work uses rodents. Researchers reported that growth hormone/STAT5 signalling within POMC neurons regulated glucoprivic hyperphagia in mice, linking a hormonal input to POMC-neuron control of feeding (PMID 31494175). A separate mouse study reported that deficiency of CAPS2, a protein involved in dense-core vesicle secretion, caused POMC accumulation in the pituitary and altered food intake behaviour (PMID 32891671). Synaptic inputs to these cells also appear context-dependent: the study of endocannabinoid signalling at hypothalamic steroidogenic factor-1/POMC synapses reported that this signalling was sensitive to both sex and diet in mice (PMID 29973869).

Glucose homeostasis

POMC neurons are studied not only for body weight but for metabolic control more broadly. A 2018 mouse study reported that hypothalamic POMC was necessary for normal glucose homeostasis in female mice, indicating sex-specific consequences of losing hypothalamic POMC expression (PMID 30283405).

Gene regulation in pituitary cells

Another research strand asks how POMC transcription itself is switched on and off. Researchers described the structural basis of the transcription factor NR4A1 bound to the human pituitary POMC gene promoter, providing a molecular picture of one regulatory interaction (PMID 31822342). Work in AtT-20 corticotroph cells reported that the co-chaperones Fkbp4 and Fkbp5 had differing effects on POMC gene regulation (PMID 34072036).

Light, skin and sensory signalling

POMC also appears in studies connecting skin exposure to central signalling. An experimental dermatology study reported that ultraviolet B stimulated POMC signalling in the arcuate nucleus of the hypothalamus in mice (PMID 26513428). In humans, a 2016 report described a biomimetic POMC peptide applied topically and reported suppression of capsaicin-induced sensory irritation in human volunteers (PMID 28003842).

POMC Deficiency and Melanocortin Pharmacology: What Studies Report

The clearest clinical illustration of POMC's role comes from rare genetic deficiency. A 2016 New England Journal of Medicine report described patients with POMC deficiency treated with the melanocortin-4 receptor agonist setmelanotide, and the authors reported weight loss and reduced hunger with treatment (PMID 27468060). That report is frequently cited as evidence that the downstream melanocortin receptor can be engaged even when the precursor peptide is absent. The verified literature summarised here does not characterise a full safety or tolerability profile for melanocortin agonists, and no dosing information beyond what the cited report describes is discussed on this page.

Limitations of the Evidence

Most mechanistic findings above come from mice or from cultured pituitary cells, and researchers reported sex- and diet-dependent differences in several of them (PMID 29973869), so results do not transfer automatically to humans. Human data in this collection are narrow: a rare-disease treatment report (PMID 27468060) and a small sensory-irritation study of a topical biomimetic peptide (PMID 28003842). Nothing here establishes outcomes in healthy people, and none of it constitutes guidance for any individual.
