What Is IGF-1 LR3 Studied For? Research Areas, Per the Literature
IGF-1 LR3 is a long R3 analogue of insulin-like growth factor 1 that appears in laboratory research rather than in approved medicine. Published work clusters into fetal sheep physiology, pancreatic islet and insulin secretion studies, a rat sciatic nerve regeneration scaffold, an amyloid mouse model, and cell culture signalling. Human clinical trials of IGF-1 LR3 as a treatment are absent from this evidence set. IGF-1 LR3 is not approved by the FDA as a medicine for any condition. This page is not guidance.
IGF-1 LR3 is not approved by the FDA as a medicine for any condition. This page is not guidance. The sections below describe what published studies have examined, the model or population each study used, and what the authors reported. They do not describe how any substance is administered outside a laboratory.
IGF-1 LR3, also written as long R3 IGF-1, is a modified version of insulin-like growth factor 1 carrying an arginine substitution at position 3 and a 13 amino acid N-terminal extension. That structure reduces binding to IGF binding proteins, which is why the analogue has been adopted as a research reagent and as a tool compound in animal physiology. A 2026 review in Frontiers in Endocrinology placed IGF-1 analogues within the broader group of peptides that modulate the growth hormone and IGF-1 axis and described a gap between the clinical evidence base and unsupervised self-administration (PMID 42395176).

Fetal Growth Physiology: the Largest Body of IGF-1 LR3 Animal Work
The most developed line of IGF-1 LR3 research sits in fetal sheep physiology, where the analogue has been infused to ask whether raising IGF-1 signalling changes fetal growth, organ size and placental nutrient handling. In a 2021 study in fetal sheep, researchers reported that IGF-1 infusion increased organ growth but that the increase was not explained by stimulated nutrient transfer to the fetus (PMID 33427051). The distinction matters because it separates a direct tissue-level effect from an indirect placental supply effect.
A 2025 study examined a different and more clinically relevant model, late-gestation growth-restricted fetal sheep, and the study found that IGF-1 LR3 did not promote growth in those fetuses (PMID 39679943). Read together, these two animal reports show that an effect observed in one fetal sheep context did not transfer to a growth-restricted context, which is a common pattern when a signalling molecule is tested against an underlying nutrient or placental constraint (PMID 33427051, PMID 39679943).
What this area does not establish
These are sheep fetal physiology experiments using continuous infusion under laboratory monitoring. They were not trials of a therapy for human growth restriction, and the 2025 report was explicitly negative for growth promotion in the growth-restricted model (PMID 39679943).

Try it freePancreatic Islet Function and Glucose-Stimulated Insulin Secretion
A second research area uses IGF-1 and IGF-1 LR3 to probe how growth factor signalling interacts with the developing endocrine pancreas. In late gestation fetal sheep, researchers reported that glucose-stimulated insulin secretion was reduced after a one week IGF-1 infusion and attributed the reduction to an intrinsic islet defect rather than to the systemic environment (PMID 33938236).
A 2023 study then tested an acute exposure and reported that glucose-stimulated insulin secretion was attenuated during an acute IGF-1 LR3 infusion into fetal sheep, but that the attenuation did not persist once islets were isolated and studied outside the animal (PMID 37114757). The contrast between the acute and the one week findings is informative for anyone reading this literature: duration of exposure changed whether the islet carried the change with it (PMID 37114757, PMID 33938236).
Peripheral Nerve Regeneration Scaffolds
IGF-1 LR3 has also been studied as a cargo molecule inside tissue engineering constructs rather than as a systemic agent. A 2025 report in International Journal of Biological Macromolecules described a decellularized Alstroemeria stem-based nerve conduit combined with GelMA hydrogel and controlled IGF-1 LR3 release, and the authors reported enhanced sciatic nerve regeneration in rats (PMID 41015370). In that design the peptide was one component of a device, so the reported outcome belongs to the combined conduit and release system in a rat model, not to the peptide considered on its own (PMID 41015370).

A 2025 study in male 5XFAD mice, a transgenic amyloid model of Alzheimer disease, tested intranasal long R3 IGF-1 and reported that the treatment promoted amyloid plaque remodeling in the cerebral cortex but failed to preserve cognitive function (PMID 39610283). That dissociation between a pathology marker and a behavioural outcome is the central finding of the paper, and it is the kind of result that argues against treating plaque changes as a stand-in for cognitive benefit (PMID 39610283). No human trial of IGF-1 LR3 in Alzheimer disease appears in this citation set.
Cell Signalling and Laboratory Use
Outside whole-animal work, IGF-1 and its analogues are studied as signalling ligands in cultured cells. A 2022 in vitro study using Chinese hamster ovary cells reported that N-linked glycosylation was critical for insulin-like growth factor 1 signalling, identifying a post-translational requirement on the receptor side of the pathway (PMID 36499281). Work of this kind explains part of why long R3 analogues are common reagents in cell culture: they are tools for interrogating the IGF-1 receptor pathway, and the study above is an example of mechanistic rather than therapeutic research (PMID 36499281).

Several topics are frequently attached to IGF-1 LR3 in general discussion but are not supported by the verified literature on this page.
- Human muscle growth or body composition. No human trial of IGF-1 LR3 for muscle mass, strength or fat loss appears in this citation set. The only human-facing source here is a review that framed GH-IGF1 axis peptides as a field where clinical evidence lags behind self-administration (PMID 42395176).
- Athletic performance. The same 2026 review discussed performance-enhancing peptides modulating the GH-IGF1 axis and described the evidence gap rather than demonstrating performance outcomes (PMID 42395176).
- Treatment of fetal growth restriction. Evidence here is preclinical and negative in the growth-restricted sheep model (PMID 39679943).
- Cognitive protection. Preclinical only, and the behavioural endpoint was not preserved in the mouse model tested (PMID 39610283).
- Human nerve injury repair. The regeneration evidence is a rat model of a combined conduit device, not a human study (PMID 41015370).
Stating this plainly is more useful than hedging: across the papers listed here, the IGF-1 LR3 literature is animal and in vitro, and it includes clearly negative or partially negative results (PMID 39679943, PMID 39610283).
Safety and Physiological Signals in Animal Research: What Studies Report
The verified papers were not safety trials, so they do not provide an adverse event profile. They do contain physiological observations relevant to risk discussion. Researchers reported reduced glucose-stimulated insulin secretion after a one week IGF-1 infusion in late gestation fetal sheep and traced it to an intrinsic islet defect (PMID 33938236), and a separate study reported attenuated glucose-stimulated insulin secretion during acute IGF-1 LR3 infusion in fetal sheep that did not persist in isolated islets (PMID 37114757). Because IGF-1 shares signalling with insulin pathways, these islet findings are the most frequently cited physiological caution in this animal literature (PMID 37114757). The 2026 review additionally addressed unsupervised use of GH-IGF1 axis peptides as a clinical concern (PMID 42395176). No human adverse event dataset for IGF-1 LR3 appears in this citation set.

- Check the model first. Fetal sheep, rat sciatic nerve and transgenic mice answer different questions, and none of them is a human population (PMID 33427051).
- Check whether the peptide acted alone. In the nerve conduit study the peptide was embedded in a scaffold with controlled release (PMID 41015370).
- Check whether a surrogate marker matched a functional outcome. Plaque remodeling changed while cognition did not (PMID 39610283).
- Check exposure duration. Acute and one week infusions produced different islet results in sheep (PMID 37114757, PMID 33938236).

Frequently asked questions
What has IGF-1 LR3 actually been studied for?▾
Published work falls into a few clusters: fetal sheep growth and organ physiology (PMID 33427051), growth restriction (PMID 39679943), pancreatic islet and insulin secretion physiology (PMID 33938236), a rat sciatic nerve conduit with controlled peptide release (PMID 41015370), an amyloid mouse model (PMID 39610283), and cell culture signalling (PMID 36499281). These are animal and in vitro studies, not human treatment trials.
Are there human clinical trials of IGF-1 LR3?▾
No human trial of IGF-1 LR3 as a treatment appears in the verified papers summarised here. The only human-facing source is a 2026 review of peptides modulating the growth hormone and IGF-1 axis, which described a gap between clinical evidence and patient self-administration rather than reporting trial outcomes (PMID 42395176). IGF-1 LR3 is not approved by the FDA as a medicine for any condition.
Did IGF-1 LR3 increase growth in animal studies?▾
Results differed by model. Researchers reported that IGF-1 infusion to fetal sheep increased organ growth, though not through stimulated nutrient transfer to the fetus (PMID 33427051). In a separate 2025 study, IGF-1 LR3 did not promote growth in late-gestation growth-restricted fetal sheep (PMID 39679943). The two findings are not interchangeable because the models differed.
What did studies find about insulin secretion?▾
In late gestation fetal sheep, researchers reported that glucose-stimulated insulin secretion was reduced after a one week IGF-1 infusion and attributed it to an intrinsic islet defect (PMID 33938236). A 2023 study reported that attenuated secretion during an acute IGF-1 LR3 infusion did not persist once islets were isolated (PMID 37114757). Exposure duration appeared to matter.
Is there evidence for nerve repair?▾
The available evidence is preclinical and device based. A 2025 study described a decellularized Alstroemeria stem-based conduit combined with GelMA and controlled IGF-1 LR3 release, and the authors reported enhanced sciatic nerve regeneration in rats (PMID 41015370). The outcome belongs to the combined construct in a rat model, and no human nerve repair study appears in this citation set.
What about Alzheimer disease or cognition?▾
One animal study tested intranasal long R3 IGF-1 in male 5XFAD mice and reported that it promoted amyloid plaque remodeling in the cerebral cortex but failed to preserve cognitive function (PMID 39610283). That dissociation between a pathology marker and a behavioural outcome is the key result. No human cognition trial of IGF-1 LR3 appears in the verified literature here.
Why is IGF-1 LR3 common in cell culture research?▾
Long R3 analogues are used as ligands for interrogating the IGF-1 receptor pathway in laboratory cells. A 2022 in vitro study in Chinese hamster ovary cells reported that N-linked glycosylation was critical for insulin-like growth factor 1 signalling (PMID 36499281). Research of that type is mechanistic, addressing how the pathway works rather than whether a peptide treats a condition.
