What Is Follistatin Studied For? Research Areas, Per the Literature
Follistatin is a protein that binds activins and related ligands such as myostatin. Published work sits in several buckets: gene transfer studies in muscular dystrophy, exercise and sarcopenia studies measuring follistatin as a marker, fusion protein work in mice, metabolic studies in diet induced obese mice, tumor biology, inflammation reviews, reproductive biology and tissue repair models. Most findings come from cells and animals. Human data are small and early. This page summarises what the studies reported, not what anyone should do.
Short answer: what the literature covers
Follistatin is not approved by the FDA as a medicine for any condition. This page is not guidance. The question "what is follistatin used for" is answered here as "what has follistatin been studied for," because the published record is made up of laboratory experiments, animal models, observational tissue studies and a very small number of early human trials. The research areas below are organised by field, and each statement carries the study type, the model or population, and a PubMed link in the same passage.

What follistatin is in biological terms
Follistatin is a secreted glycoprotein that binds and neutralises members of the transforming growth factor beta superfamily, including activins and myostatin. A review of activin biology described activins and their binding proteins, follistatin included, as active participants in inflammatory signalling rather than passive bystanders, and summarised their roles across immune and repair processes (PMID 19261538). That ligand trapping role is the mechanistic reason follistatin appears in such different research fields at once: muscle, fat tissue, reproduction, inflammation and cancer all involve activin family signalling.
Research area 1: muscular dystrophy and gene transfer
The most direct human work on follistatin as an intervention is gene transfer. A phase 1/2a follistatin gene therapy trial in participants with Becker muscular dystrophy reported safety and functional outcomes, including six minute walk test distance, after intramuscular delivery of a follistatin gene construct, and researchers framed the work as an early stage test of feasibility and tolerability rather than a definitive efficacy study (PMID 25322757). The design was open label and small, which is standard for first in human gene transfer but means the results cannot establish benefit.
Preclinical work has combined follistatin with other gene products. A study in an aged Duchenne muscular dystrophy mouse model reported that co-delivery of micro-dystrophin and follistatin restored muscle function measures better than the comparator conditions described by the researchers in that animal model (PMID 23863459). That is an animal finding in a genetic disease model and does not transfer automatically to humans or to people without dystrophinopathy.

A large share of the follistatin literature does not administer follistatin at all. Instead, follistatin is measured as a circulating or tissue biomarker that moves with exercise and aging. A randomized controlled trial in older adult women with sarcopenia examined 16 weeks of resistance training and reported changes in muscle quality alongside muscle growth factor measures (PMID 34201810). That is a human trial of exercise, not of follistatin as a product.
Animal work has compared training modes directly. A 2025 study in aged female rats examined high intensity interval training and resistance training and reported follistatin and myostatin levels in gastrocnemius muscle, framing the balance between the two proteins as the outcome of interest (PMID 41359200). Again, the intervention was exercise and follistatin was the readout.
Nutrition and supplement trials have used similar muscle endpoints. A 12 week randomized, double blind, placebo controlled clinical trial of pasteurized Akkermansia muciniphila HB05 reported improvements in muscle strength and function measures in the enrolled adult population (PMID 39683431). This trial is relevant context for how muscle endpoints are measured in humans, and it did not test follistatin as an administered agent.
Research area 3: engineered follistatin proteins in mice
Protein engineering is a distinct research area. A methods paper described the preparation of a monovalent follistatin-like 3 Fc fusion protein and the evaluation of its effects on muscle mass in mice, giving step by step procedures for producing the construct and testing it in an animal model (PMID 34585166). Work of this kind is aimed at other laboratories reproducing the method. It is preclinical, it uses a follistatin-like 3 fusion rather than native follistatin, and it reports mouse outcomes only.

Follistatin has been studied in fat tissue biology. In a mouse study, intraperitoneal administration of follistatin in high-fat diet induced obese mice promoted adipocyte browning, and researchers interpreted the result as evidence that follistatin can influence adipose tissue phenotype in that model (PMID 31365569). This is an animal study in a diet induced obesity model. No equivalent human trial of administered follistatin for body composition appears in the verified literature on this page.
Research area 5: tumor biology and the tumor microenvironment
Follistatin appears frequently in oncology research, mostly as an expression marker rather than a treatment. A study of human invasive breast tumors examined follistatin expression in tumor tissue and reported pathologic and clinical associations with that expression (PMID 27389553). That is an observational human tissue study, so it describes association rather than cause.
A 2024 review summarised follistatin across tumors and their microenvironment and discussed implications for drug resistance, placing follistatin within activin and TGF beta signalling in cancer biology (PMID 38392348). Reviews of this type synthesise mixed preclinical and observational data, and the presence of follistatin in cancer signalling literature is a reason the biology receives careful scrutiny rather than a claim of harm or benefit.

Activin and follistatin biology overlaps with inflammation. The review on activin and related proteins in inflammation described follistatin as part of a regulatory system influencing inflammatory responses and tissue repair, rather than an inert binding protein (PMID 19261538). This area is largely mechanistic and preclinical.
Research area 7: reproductive biology and embryo development
Follistatin was originally characterised in reproductive endocrinology, and developmental work continues. A study on pig parthenogenetic embryos examined the effect of follistatin on pre-implantation development in vitro and reported developmental outcomes in that embryo culture system (PMID 29119699). This is laboratory animal embryology and has no stated human application.

Follistatin also appears in regeneration research outside muscle. In a mouse study, follistatin expressed in mechanically damaged salivary glands induced proliferation of CD49f positive cells, and researchers linked follistatin expression to the repair response in that tissue (PMID 33203957). The finding is an animal observation about endogenous follistatin in an injury model.

Several popular framings of follistatin are not matched by the published record summarised here.
- Muscle growth in healthy adults. The human intervention data in this citation set come from a small early phase gene transfer trial in a muscular dystrophy population (PMID 25322757). Muscle mass work with engineered follistatin-like proteins remains in mice (PMID 34585166).
- Fat loss or body recomposition. The adipocyte browning finding is from obese mice given follistatin intraperitoneally (PMID 31365569). No human trial of administered follistatin for body composition appears in this verified set.
- Anti-aging claims. Aging related work here is exercise physiology in older women (PMID 34201810) and aged rats (PMID 41359200), where follistatin was measured rather than given.
- Fertility applications. The developmental data are in vitro pig embryo experiments (PMID 29119699), not human reproductive trials.
Where a research area is represented only by cells or animals, the honest description is that the hypothesis is open, not that an effect has been shown in people.
Adverse Events and Safety Signals: What Studies Report
Human safety information in this citation set is limited to one early phase study. The phase 1/2a follistatin gene therapy trial in Becker muscular dystrophy was designed with safety as a primary focus and reported safety outcomes alongside functional measures in a small enrolled group (PMID 25322757). A trial of that size cannot detect uncommon events, and its findings apply to the specific construct and delivery used by the researchers.
The theoretical concerns discussed in the literature are mechanistic. Because follistatin sits inside activin and TGF beta signalling, a 2024 review examined follistatin in tumors and their microenvironment and discussed links to drug resistance (PMID 38392348), while a human tissue study reported follistatin expression associations in invasive breast tumors (PMID 27389553). Associations in tumor tissue are not evidence that administered follistatin causes cancer, and they are also not reassurance; they are a reason the biology is studied carefully. Inflammation reviews make the same general point that activin family signalling is broad rather than muscle specific (PMID 19261538).

- Check whether follistatin was administered or merely measured. Exercise trials such as the 16 week resistance training study in older women with sarcopenia measured it (PMID 34201810).
- Check the species. Adipocyte browning was reported in mice (PMID 31365569) and muscle function restoration in a mouse dystrophy model (PMID 23863459).
- Check the molecule. Follistatin, follistatin-like 3 and Fc fusion constructs are not interchangeable (PMID 34585166).
- Check the population. Findings in a dystrophinopathy population (PMID 25322757) do not generalise to healthy adults.
Bottom line
Follistatin is not approved by the FDA as a medicine for any condition. This page is not guidance. The published record describes follistatin as a research protein studied in muscular dystrophy gene transfer (PMID 25322757), in exercise and aging muscle biology (PMID 41359200), in metabolic animal models (PMID 31365569), in tumor and inflammation signalling (PMID 38392348) and in developmental and repair models (PMID 33203957). Frequently asked questions
Is follistatin an approved treatment for anything?▾
No. Follistatin is not approved by the FDA as a medicine for any condition, and this page is not guidance. The closest human work is an early phase 1/2a follistatin gene therapy trial in Becker muscular dystrophy, which reported safety and functional outcomes in a small group (PMID 25322757). Early phase trials test feasibility and tolerability rather than establishing benefit.
What has follistatin been studied for in muscle research?▾
Two different things. In animals, co-delivery of micro-dystrophin and follistatin restored muscle function measures in an aged Duchenne muscular dystrophy mouse model (PMID 23863459), and a follistatin-like 3 Fc fusion protein was evaluated for effects on muscle mass in mice (PMID 34585166). In humans, follistatin is usually measured as a marker during exercise trials rather than administered (PMID 34201810).
Does follistatin affect fat tissue?▾
In mice. Researchers reported that intraperitoneal administration of follistatin promoted adipocyte browning in high-fat diet induced obese mice (PMID 31365569). That is an animal study in a diet induced obesity model. No human trial of administered follistatin for body composition appears in the verified literature summarised on this page, so the question remains open in people.
Why does follistatin appear in cancer research?▾
Because it sits within activin and TGF beta signalling. A study of human invasive breast tumors reported follistatin expression with pathologic and clinical associations (PMID 27389553), and a 2024 review discussed follistatin in tumors and their microenvironment with implications for drug resistance (PMID 38392348). Those are observational and review findings about tumor biology, not evidence that administered follistatin causes or treats cancer.
Do exercise studies change follistatin levels?▾
Exercise studies measure it. A randomized controlled trial examined 16 weeks of resistance training in older adult women with sarcopenia and reported muscle quality alongside muscle growth factor measures (PMID 34201810). In aged female rats, researchers compared high intensity interval training and resistance training and reported follistatin and myostatin levels in gastrocnemius muscle (PMID 41359200).
Is follistatin studied outside muscle and metabolism?▾
Yes. A review described activins and their binding proteins, including follistatin, as active participants in inflammatory signalling (PMID 19261538). In reproductive biology, follistatin was tested on pre-implantation development of pig parthenogenetic embryos in vitro (PMID 29119699). In tissue repair, follistatin expressed in mechanically damaged mouse salivary glands induced proliferation of CD49f positive cells (PMID 33203957).
What safety information exists in the human literature?▾
Very little. The phase 1/2a follistatin gene therapy trial in Becker muscular dystrophy reported safety outcomes alongside functional measures in a small enrolled group (PMID 25322757), which is too small to detect uncommon events. Mechanistic concerns raised in reviews relate to the breadth of activin family signalling across inflammation and tumor biology (PMID 19261538, PMID 38392348).
