IGF-1 vs Insulin: Mechanism and Class Compared

IGF-1 vs Insulin: Mechanism and Class Compared
The short answer

IGF-1 is not insulin, but the two belong to the same ancient peptide superfamily and signal through closely related tyrosine kinase receptors. Published pharmacology describes insulin acting mainly through the insulin receptor on metabolic tissues, and IGF-1 acting mainly through IGF-1R, with hybrid receptors and cross-binding blurring the line. The literature also separates them by carrier proteins, upstream control, and regulatory category. This page summarises what studies report and does not compare them as options.

The short version: same family, different molecules

Questions about whether IGF-1 "is" insulin come from a real structural fact: insulin-like growth factor 1 and insulin are homologous peptides descended from a common ancestral molecule, and their receptors are homologous tyrosine kinases. That shared ancestry is why the pathway is usually written in the literature as a single unit, the insulin/IGF-1 signalling (IIS) axis, when authors discuss conserved biology. A review of conserved longevity pathways across species described insulin/IGF-1 signalling as one of the most evolutionarily preserved nutrient-sensing systems studied in model organisms (PMID 31109448). Work in Caenorhabditis elegans has treated the same axis as a single conserved pathway, with the DAF-2 receptor acting as the worm counterpart of the mammalian insulin and IGF-1 receptors in studies of immune ageing (PMID 36380393).

Homology is not identity. In mammals the two peptides are produced by different tissues, controlled by different upstream signals, carried in blood in different ways, and bound with very different affinities by two related but distinct receptors. The sections below set out where the published pharmacology places them together and where it separates them. This page is for educational purposes only and is not medical advice; consult a licensed physician for any health question. For background on the growth factor itself, see the IGF-1 overview.

Structure and peptide class

Both molecules are small polypeptides built on the same insulin fold: A- and B-domain segments held together by disulfide bonds. Mature insulin is processed from proinsulin with removal of the connecting C-peptide, leaving a two-chain molecule. IGF-1 retains an analogous C-domain and adds a D-domain extension, so it circulates as a single chain. This structural difference is the main reason the two peptides are classed separately, insulin as a metabolic hormone, IGF-1 as a growth factor, even though reviews of their signalling describe overlapping intracellular machinery (PMID 34208601).

A further structural consequence concerns transport. Insulin circulates essentially free and is cleared quickly. IGF-1 circulates almost entirely bound to IGF-binding proteins, which reviews describe as regulating its availability to receptors; the same reviews note that IGF-binding protein status shapes how much free growth factor reaches tissue receptors in disease states such as renal cell carcinoma (PMID 27405474). Insulin has no equivalent carrier system.

Receptor targets: IR, IGF-1R and hybrids

The cleanest way the literature distinguishes the two is by receptor preference rather than by downstream pathway. Insulin binds the insulin receptor (IR) with high affinity and the IGF-1 receptor with much lower affinity; IGF-1 does the reverse. A review of insulin analogs and cancer biology examined exactly this issue, reporting that modified insulin molecules can differ from native insulin in their relative affinity for IGF-1R and in their mitogenic versus metabolic signalling profile, which is why regulatory assessment of analogs has focused on receptor-binding characterisation (PMID 22649410).

The insulin receptor itself exists in two splice isoforms, IR-A and IR-B, and the review of insulin analogs described IR-A as the isoform associated with mitogenic signalling and with binding of IGF-2 in addition to insulin (PMID 22649410). Because IR and IGF-1R are homologous half-receptors, they also assemble into hybrid receptors containing one IR hemireceptor and one IGF-1R hemireceptor. A review of endothelial IGF-1 receptor signalling in diabetes and insulin resistance discussed hybrid receptor formation in the vasculature and reported that the balance of IGF-1R, IR and hybrids influences endothelial insulin sensitivity and nitric oxide signalling (PMID 26712712). That hybrid biology is the strongest reason the two compounds cannot be treated as pharmacologically independent.

Shared downstream machinery

Once either receptor is activated, the intracellular cascades converge. Both recruit insulin receptor substrate proteins and branch into the PI3K-AKT arm and the RAS-MAPK arm. A review of IGF-1 signalling in glucose metabolism described IGF-1R activation feeding into PI3K/AKT signalling with downstream effects on glucose handling and on proliferative signalling in colorectal cancer cells (PMID 34208601). A Science Signaling focus issue on metabolic signals framed insulin signalling as a network rather than a linear pathway, emphasising crosstalk between metabolic and growth-related outputs (PMID 21139135). The usual textbook shorthand, insulin is metabolic, IGF-1 is mitogenic, is therefore described in these reviews as a matter of emphasis and tissue context, not of separate machinery.

Upstream control: who releases what

The regulatory inputs differ sharply. Insulin is secreted by pancreatic beta cells in response to circulating glucose and other nutrient and incretin signals. IGF-1 is produced largely by the liver under growth hormone stimulation, with additional local production in many tissues acting in autocrine and paracrine fashion.

Acromegaly illustrates how tightly the two systems are coupled in vivo. A review of secondary diabetes mellitus in acromegaly reported that chronic growth hormone excess, with associated elevation of IGF-1, is accompanied by insulin resistance and a high prevalence of disordered glucose metabolism in affected patients (PMID 36882643). Researchers in that review described glucose metabolism in acromegaly as reflecting the opposing influences of growth hormone and IGF-1 on insulin sensitivity rather than a single direction of effect (PMID 36882643).

Tissue-level examples where the two overlap

Heart

A 2024 review of IGF-1 signalling in the heart described IGF-1R activation in cardiomyocytes as influencing cardiac growth, metabolism and stress responses, and discussed how IGF-1 receptor signalling intersects with insulin receptor signalling in the myocardium (PMID 38713091). The review framed cardiac IGF-1 biology as context-dependent, with physiological and pathological outcomes reported in different experimental settings (PMID 38713091).

Vascular endothelium

Endothelial cells express both receptors plus hybrids, and the review of endothelial IGF-1R signalling reported that altering IGF-1R abundance in endothelium changed insulin-stimulated signalling and vascular responses in experimental models of insulin resistance (PMID 26712712).

Skin and sebaceous gland

A 2023 review of insulin and sebaceous gland function described insulin and IGF-1 as both acting on sebocytes, with the review reporting effects on sebaceous lipogenesis and differentiation and discussing the relevance of hyperinsulinaemia to sebaceous gland activity (PMID 37727660). This is one of the clearer examples in the literature of two distinct ligands converging on one tissue response.

Infection biology

A review of the IGF-1 signalling pathway in viral infections described viral manipulation of IGF-1R-linked PI3K/AKT signalling in host cells, reporting that several virus families exploit this pathway during replication (PMID 34452353).

Proliferation and cancer biology: what studies report

Because the shared PI3K/AKT and MAPK arms are growth-promoting, much of the comparative literature sits in oncology. A review of obesity, type 2 diabetes and cancer risk reported that hyperinsulinaemia and altered IGF axis signalling are among the mechanisms proposed to link metabolic disease with cancer incidence (PMID 33604295). The colorectal cancer review described IGF-1 signalling as interacting with tumour glucose metabolism, connecting a growth factor pathway to a classically insulin-governed process (PMID 34208601). In renal cell carcinoma, researchers reported that IGF-1 signalling components and binding protein status were relevant to tumour cell behaviour (PMID 27405474).

Safety signals in the comparative literature: What Studies Report

Published safety discussion of these two molecules is mostly mechanistic and epidemiological rather than drawn from head-to-head human trials. The review of insulin analogs and cancer examined whether analogs with altered IGF-1R affinity carry different mitogenic risk, and reported that this question drove both laboratory receptor studies and observational analyses (PMID 22649410). The obesity and cancer review reported hyperinsulinaemia as a proposed contributor to cancer risk in metabolic disease (PMID 33604295). On the growth hormone side, the acromegaly review reported a high burden of secondary diabetes in states of chronic GH and IGF-1 excess (PMID 36882643). None of these findings describe outcomes of non-clinical use, and the study designs behind them do not support extrapolation to healthy individuals.

Regulatory and classification differences

Insulin products are approved prescription medicines for diabetes mellitus and are regulated as biologics in the United States. Recombinant human IGF-1 (mecasermin) is an approved prescription product with a far narrower indication, severe primary IGF-1 deficiency, and is not interchangeable with insulin in any regulatory sense. Other IGF-1 related materials sold for laboratory work carry research-use-only labelling, which means they are not authorised for human administration. The classification difference matters: the two peptides sit in different therapeutic categories despite sharing a signalling pathway that reviews describe as unified across species (PMID 31109448).

How to read the comparison

No paper in this set compares the two molecules as alternatives, and nothing here should be read as ranking them. The literature treats them as two arms of one conserved signalling system with different physiological jobs.

Frequently asked questions

Is IGF-1 a form of insulin?▾

No. IGF-1 is a separate peptide that shares structural homology and a related receptor with insulin, which is why reviews describe a combined insulin/IGF-1 signalling axis conserved across species (PMID 31109448). IGF-1 keeps a single-chain structure and circulates bound to IGF-binding proteins, a carrier system insulin lacks (PMID 27405474). The two are relatives, not the same molecule.

Do IGF-1 and insulin activate the same receptor?▾

Each has a preferred receptor, but cross-binding exists. A review of insulin analogs reported that insulin receptor isoforms and IGF-1R differ in ligand affinity and in mitogenic versus metabolic output, which is why analog receptor binding is characterised closely (PMID 22649410). Endothelial cells additionally form IR/IGF-1R hybrid receptors, reported to influence insulin-stimulated vascular signalling (PMID 26712712).

Why is the pathway often written as "insulin/IGF-1 signalling"?▾

Because the two converge on shared intracellular machinery. Reviews of conserved longevity pathways describe insulin/IGF-1 signalling as a single nutrient-sensing system preserved across species (PMID 31109448), and work in Caenorhabditis elegans treats the DAF-2 receptor as the shared counterpart of both mammalian receptors in studies of immune ageing (PMID 36380393).

What controls IGF-1 release compared with insulin?▾

Insulin secretion follows glucose and nutrient signalling, framed in a Science Signaling focus issue as a branching metabolic network rather than a linear pathway (PMID 21139135). IGF-1 is produced mainly by the liver under growth hormone stimulation. A review of acromegaly reported that chronic growth hormone and IGF-1 excess is accompanied by insulin resistance and frequent secondary diabetes (PMID 36882643).

Does the literature link either molecule to cancer biology?▾

Both appear in that literature mechanistically. A review of obesity and type 2 diabetes reported hyperinsulinaemia and altered IGF axis signalling among proposed links to cancer risk (PMID 33604295). IGF-1 signalling has also been reviewed in colorectal cancer glucose metabolism (PMID 34208601) and in renal cell carcinoma (PMID 27405474). These are mechanistic and epidemiological reviews, not trials in healthy people.

Do IGF-1 and insulin overlap in specific tissues?▾

Yes, in several. A 2023 review reported that both insulin and IGF-1 act on sebocytes with effects on sebaceous lipogenesis (PMID 37727660). A 2024 cardiac review described IGF-1R signalling in cardiomyocytes intersecting with insulin receptor signalling (PMID 38713091), and a virology review reported viral exploitation of IGF-1R-linked PI3K/AKT signalling (PMID 34452353).

Are IGF-1 and insulin regulated the same way?▾

No. Insulin products are approved prescription medicines for diabetes, while recombinant human IGF-1 is approved only for a narrow indication, severe primary IGF-1 deficiency. Materials labelled research-use-only are not authorised for human administration. The classification gap persists even though reviews describe a shared, evolutionarily conserved signalling pathway behind both molecules (PMID 31109448).
