How Does FGF21 Work? Mechanism of Action, Per the Research

How Does FGF21 Work? Mechanism of Action, Per the Research
The short answer

FGF21 is a liver-derived endocrine hormone that signals through fibroblast growth factor receptors only when the co-receptor beta-Klotho is present, which restricts its activity to a limited set of tissues such as adipose tissue and parts of the brain. Published pharmacology reviews describe downstream effects on glucose handling, lipid metabolism and energy balance, while animal and cell studies map specific circuits and tissue targets. Much of the mechanistic detail comes from rodent and in vitro work rather than from human physiology.

Fibroblast growth factor 21 (FGF21) is an endocrine member of the fibroblast growth factor family. Unlike classical fibroblast growth factors that act locally on neighbouring cells, reviews of its pharmacology described FGF21 as a circulating hormone released mainly by the liver that acts on distant tissues through fibroblast growth factor receptors paired with the co-receptor beta-Klotho (PMID 33758421). That co-receptor requirement is the single most important point in any mechanistic description of FGF21, because it explains why a hormone with receptors on nearly every cell type only produces effects in a narrow set of tissues. A broader background on the molecule itself is covered in the FGF21 overview.

Receptor Targets: FGFR1c Plus Beta-Klotho

Reviews of FGF21 biology reported that FGF21 binds fibroblast growth factor receptors of the "c" splice variant class, principally FGFR1c, and that this binding requires beta-Klotho (encoded by KLB) as an obligate co-receptor to form a productive signalling complex (PMID 33758421). A separate review of the therapeutic landscape described the same receptor and co-receptor arrangement as the basis for engineering FGF21 analogues and FGFR1c/beta-Klotho agonist antibodies that mimic the native ligand (PMID 32764725).

Why the Co-Receptor Determines Tissue Selectivity

Because beta-Klotho expression is limited rather than universal, the tissues capable of responding to FGF21 are correspondingly limited. Review literature on FGF21 described white and brown adipose tissue, the liver and defined regions of the central nervous system as the principal beta-Klotho expressing targets, and framed this distribution as the reason FGF21 produces a coordinated metabolic response rather than a generalised growth factor response (PMID 33758421). A review focused on type 2 diabetes made the related point that this receptor selectivity has been treated as an attractive feature in drug design, since it narrows where a systemically delivered agonist can act (PMID 27031294).

After Binding: Signalling and Intracellular Trafficking

Once the FGF21, FGFR1c and beta-Klotho complex forms, reviews described receptor dimerisation and activation of the canonical fibroblast growth factor receptor tyrosine kinase cascade, including mitogen activated protein kinase signalling, as the proximal intracellular event that leads to changes in gene expression in responsive tissues (PMID 33758421). The therapeutic review placed the same cascade at the centre of attempts to engineer longer acting analogues with preserved receptor engagement (PMID 32764725).

Receptor engagement is only part of the picture. A 2019 eLife study used cryo-electron tomography combined with gold nanoparticle labelling to visualise FGF21 trafficking inside intact human cells, and the researchers reported that this approach resolved where labelled FGF21 was located within cells rather than relying on indirect markers (PMID 30688648). That work is cell-based imaging, not a clinical finding, and it describes how the ligand is handled after it meets the cell surface.

Where FGF21 Comes From: Stress Driven Expression

FGF21 is not secreted at a constant rate. Reviews characterised it as a stress and nutrient responsive hormone whose hepatic expression rises under conditions such as fasting, nutrient stress and specific dietary challenges, with transcriptional control involving stress responsive factors (PMID 33758421). A 2026 in vitro study in human HepG2 hepatocytes reported that the natural compound osthole increased FGF21 expression by mediating activation of the transcription factor ATF4, which is consistent with the integrated stress response acting as one upstream driver of FGF21 transcription (PMID 41596648). That result came from a cell line, not from animals or humans.

Other organs can also drive hepatic FGF21. A 2023 Science Advances study described an interleukin 6, STAT3, mineralocorticoid receptor and FGF21 axis mediating heart to liver cross-talk after myocardial infarction, placing hepatic FGF21 induction downstream of signals originating in injured heart tissue (PMID 37018396). Hepatic nutrient sensing pathways that operate alongside FGF21 biology have also been examined directly: a 2025 Journal of Hepatology study reported that protein phosphatase 6 regulated metabolic dysfunction associated steatohepatitis through the mTORC1 pathway (PMID 39947331).

Central Nervous System Actions in Animal Work

A substantial part of the proposed mechanism of FGF21 is neural rather than purely peripheral. A 2025 Cell Metabolism study reported that FGF21 reversed metabolic dysfunction associated steatohepatitis through coordinated actions on the central nervous system and the liver, indicating that the brain component was not incidental to the hepatic outcome in that preclinical model (PMID 40367940).

Circuit level work has gone further. A 2022 Cell Metabolism study reported that FGF21 suppressed alcohol consumption through an amygdalo-striatal circuit, identifying a defined brain pathway rather than a diffuse central effect (PMID 35108517). Review literature placed such central actions within the broader description of FGF21 as a hormone that influences energy balance and macronutrient preference through beta-Klotho expressing neurons (PMID 33758421). These are animal findings and have not been shown here to translate to human behaviour.

Peripheral Tissue Actions: Fat, Liver and Heart

Adipose Tissue

A 2018 Nature Communications study reported that FGF21 increased insulin sensitivity through specific expansion of subcutaneous fat, which positioned adipose remodelling as a mediator of the systemic metabolic effect rather than a side consequence of it (PMID 29348470). Reviews of FGF21 pharmacology similarly described adipose tissue as a primary site of action, with adiponectin and adipose derived signals discussed as candidate intermediates between the receptor complex and whole body glucose handling (PMID 33758421).

Liver

Hepatic effects have been described in both directions: the liver is the main source of circulating FGF21 and also a target tissue. The 2025 Cell Metabolism study reported that the hepatic response formed part of a coordinated central and peripheral programme in a steatohepatitis model (PMID 40367940), while the therapeutic review summarised lipid and liver fat related findings as a central reason FGF21 analogues entered clinical development for metabolic disease (PMID 32764725).

Heart and Mitochondria

A 2022 Circulation study reported that an FGF21 to Sirtuin 3 axis conferred protective effects of exercise against diabetic cardiomyopathy by governing mitochondrial integrity in a preclinical model (PMID 36134579). That work links FGF21 signalling to a downstream mitochondrial regulator rather than to glucose handling alone, and it remains animal and cell level evidence.

Signalling Trade-Offs and Unwanted Effects: What Studies Report

FGF21 signalling is not uniformly favourable in every context. A 2024 Cell Metabolism study reported that tumour-secreted FGF21 acted as an immune suppressor by rewiring cholesterol metabolism of CD8 positive T cells, which illustrates that the same pathway can be co-opted in disease settings (PMID 38309268). The therapeutic review discussed the gap between preclinical promise and clinical translation for FGF21 based agents, including the pharmacokinetic limitations of native FGF21 that motivated engineered analogues (PMID 32764725). A diabetes focused review likewise framed FGF21 as an emerging target under investigation rather than a settled therapy (PMID 27031294). The verified literature summarised on this page does not establish a safety profile for FGF21 or FGF21 analogues in healthy people.

Model Caveats: What the Evidence Base Is Made Of

Several of the mechanistic studies cited here were conducted in animals or in cultured cells, and the distinction matters when reading mechanism claims:

Receptor level pharmacology is the most consistently supported part of the mechanism across reviews (PMID 33758421), while circuit specific and tissue specific details are best read as mechanistic hypotheses supported by particular models (PMID 40367940).

Frequently asked questions

What receptor does FGF21 act on?▾

Reviews of FGF21 pharmacology described signalling through fibroblast growth factor receptors of the c splice class, chiefly FGFR1c, in complex with the co-receptor beta-Klotho (PMID 33758421). A therapeutic review described the same FGFR1c and beta-Klotho pairing as the template for engineered analogues and agonist antibodies developed for metabolic disease (PMID 32764725).

Why does FGF21 only affect certain tissues?▾

Because beta-Klotho is required for productive signalling and is expressed in a limited set of tissues, including adipose tissue, liver and specific brain regions, as described in review literature (PMID 33758421). A diabetes focused review framed that receptor selectivity as a reason FGF21 has been pursued as a target, since it narrows where a circulating agonist can act (PMID 27031294).

Is the FGF21 mechanism central, peripheral, or both?▾

Published work describes both. A 2025 study reported that FGF21 reversed metabolic dysfunction associated steatohepatitis through coordinated actions on the central nervous system and the liver (PMID 40367940), and a 2022 study reported that FGF21 suppressed alcohol consumption through an amygdalo-striatal circuit (PMID 35108517). Both findings came from preclinical models rather than human trials.

How does FGF21 relate to adipose tissue?▾

A 2018 study reported that FGF21 increased insulin sensitivity through specific expansion of subcutaneous fat, positioning adipose remodelling as a mediator of the systemic metabolic effect (PMID 29348470). Review literature also described adipose tissue as a primary beta-Klotho expressing target and discussed adipose derived signals as intermediates in whole body glucose handling (PMID 33758421).

What triggers the body to make FGF21?▾

Reviews characterised FGF21 as a stress and nutrient responsive hepatic hormone whose expression changes with metabolic challenge (PMID 33758421). A 2026 in vitro study in human HepG2 hepatocytes reported that FGF21 expression increased through activation of the transcription factor ATF4 (PMID 41596648). A 2023 study described hepatic FGF21 induction downstream of heart derived signalling after myocardial infarction (PMID 37018396).

Which parts of the FGF21 mechanism are still proposed rather than established?▾

The receptor complex itself is described consistently across reviews (PMID 33758421), while tissue specific and circuit specific mechanisms rest on individual models. Examples include an FGF21 to Sirtuin 3 axis governing mitochondrial integrity in diabetic cardiomyopathy (PMID 36134579) and a 2024 report that tumour-secreted FGF21 suppressed CD8 positive T cells by rewiring cholesterol metabolism (PMID 38309268).

Has FGF21 signalling been studied directly inside human cells?▾

Yes, at an imaging level. A 2019 eLife study used cryo-electron tomography with gold nanoparticle labelling to resolve FGF21 trafficking inside intact human cells, which researchers reported as a way to track the ligand without relying on indirect markers (PMID 30688648). That work addressed intracellular handling and is distinct from clinical evidence on outcomes in people (PMID 32764725).
