What Is Fibroblast Growth Factor 21? Definition and What Research Reports

The short answer

Fibroblast growth factor 21 (FGF21) is a secreted endocrine protein of the fibroblast growth factor family, produced mainly by the liver and also by fat, muscle and pancreas. It signals through FGF receptors together with the co-receptor beta-Klotho. In the literature it appears three ways: as an endogenous hormone studied in metabolism, as a circulating biomarker measured in disease cohorts, and as a scaffold for engineered analogues. Published work has examined its role in fat metabolism, cardiac tissue, cartilage and vascular calcification.

Plain-language definition

Fibroblast growth factor 21 (FGF21) is a secreted signalling protein belonging to the fibroblast growth factor superfamily. Unlike most members of that family, which act locally on neighbouring cells, FGF21 belongs to the small "endocrine" subgroup: it enters the bloodstream and acts on distant tissues, which is why it is usually described as a hormone rather than a classical growth factor. An overview of the molecule described FGF21 as an endocrine FGF-family protein that requires the co-receptor beta-Klotho alongside conventional FGF receptors to signal in target tissues such as adipose tissue and the nervous system (PMID 38991938). It is produced predominantly by the liver, with additional expression reported in adipose tissue, skeletal muscle and pancreas.

Is FGF21 a "peptide"?

Strictly speaking, FGF21 is a small protein rather than a short peptide: the mature human molecule is roughly 180–210 amino acids long, considerably larger than the oligopeptides that dominate much peptide literature. It is nonetheless grouped with peptide and protein therapeutics in research discussions because it is a genetically encoded, secreted polypeptide that cannot be delivered orally in its native form and because its structure has been modified by protein engineering. Researchers working on computer-aided design of stabilised FGF21 variants noted that the native molecule's limited stability is a central obstacle to laboratory and biotechnological use, and the study applied computational engineering to generate more stable variants (PMID 38379823).

Where the molecule comes from

FGF21 is encoded by the FGF21 gene and secreted principally by hepatocytes. Its expression is described in the literature as highly responsive to nutritional and metabolic states rather than fixed. Experimental work in brown adipocytes reported that activin E upregulated uncoupling protein 1 and FGF21 expression, linking FGF21 production to thermogenic programmes in brown fat (PMID 38972346). A separate line of work in adipose tissue reported that hypoxia-induced lipolysis required FGF21, indicating that the hormone participates in how fat cells release stored lipid under low-oxygen conditions (PMID 32922298).

How the term is used in peptide and metabolic research

The phrase "fibroblast growth factor 21" appears in three fairly distinct contexts in the published literature, and readers encountering the term benefit from knowing which one a given paper means.

What the published literature reports

Nutrition, macronutrients and body weight

Several reports connect FGF21 to how organisms respond to specific nutrients. A human study examined FGF21 and fructose dynamics, reporting that circulating FGF21 responded to fructose exposure in people (PMID 30338119). In rodents, researchers examined FGF21 in relation to dietary macronutrient intake in female mice, a design intended to test whether the hormone influences the balance of macronutrients selected rather than total food alone (PMID 36240865). Another mouse study reported that FGF21 was required for the weight loss induced by the glucagon-like peptide-1 receptor agonist liraglutide in male mice fed high-carbohydrate diets, placing FGF21 downstream of an established incretin pathway under particular dietary conditions (PMID 37030441).

Cardiac and vascular tissue

Preclinical cardiovascular work has examined FGF21 in injured or remodelling heart tissue. One study reported that FGF21 protected against atrial remodelling by reducing oxidative stress (PMID 34708083). Another reported that FGF21 inhibited inflammation and fibrosis after myocardial infarction through the transcription factor EGR1 (PMID 34478691). These are mechanistic animal and cell findings, not demonstrations of clinical benefit in people.

Cartilage and joint tissue

In osteoarthritis models, researchers reported that FGF21 alleviated senescence, apoptosis and extracellular matrix degradation via the SIRT1–mTOR signalling pathway (PMID 34556628). This line of work is frequently cited as evidence that FGF21 signalling extends beyond classical metabolic tissue into connective tissue biology.

FGF21 as a measured biomarker

Because FGF21 is detectable in serum, a number of clinical papers treat it as a readout rather than an intervention. A 2025 study evaluated circulating FGF21 as a prognostic and diagnostic biomarker in hepatocellular carcinoma (PMID 41399394). In kidney disease, researchers reported that FGF21 both predicted and promoted vascular calcification in haemodialysis patients (PMID 34179118).

Signals of concern in the literature: What Studies Report

Not every published finding on FGF21 is favourable, and a definition page should reflect that. In haemodialysis patients, researchers reported that FGF21 predicted and promoted vascular calcification, meaning higher concentrations were associated with calcification and the hormone also appeared to contribute to it experimentally (PMID 34179118). Elevated circulating FGF21 has also been evaluated in the context of hepatocellular carcinoma, where the study framed the hormone as a disease-associated marker rather than a protective one (PMID 41399394). Because raised endogenous FGF21 frequently accompanies metabolic and organ stress, the direction of cause and effect is not settled by observational measurement alone.

Limitations of the current evidence base

Most mechanistic findings summarised above come from rodent models or isolated cells, including the brown adipocyte, lipolysis, cardiac and cartilage work cited on this page. Findings in mice on defined diets, such as the liraglutide dependency reported in male mice fed high-carbohydrate diets (PMID 37030441), are diet- and sex-specific by design and do not automatically generalise. Human data in the verified literature here are largely observational or physiological, such as the fructose dynamics study (PMID 30338119). Native FGF21 stability remains a recognised technical constraint that engineering work has attempted to address (PMID 38379823).

Frequently asked questions

What is fibroblast growth factor 21 in one sentence?

FGF21 is a secreted endocrine protein of the fibroblast growth factor family, produced mainly by the liver and also by adipose tissue, muscle and pancreas, that circulates in blood and signals through FGF receptors together with the co-receptor beta-Klotho rather than acting only on nearby cells (PMID 38991938). It is studied as a metabolic hormone and as a measurable blood biomarker.

Is FGF21 a peptide or a protein?

It is best described as a small secreted protein rather than a short peptide, since the mature molecule runs to roughly 180 to 210 amino acids. It is discussed alongside peptide research because it is a polypeptide hormone whose limited native stability has prompted protein-engineering work, including computational design of stabilised variants (PMID 38379823).

What do metabolic studies report about FGF21?

Researchers reported that hypoxia-induced adipose lipolysis required FGF21 (PMID 32922298) and that activin E upregulated uncoupling protein 1 and FGF21 in brown adipocytes (PMID 38972346). A separate mouse study reported that FGF21 was required for weight loss induced by the GLP-1 receptor agonist liraglutide in male mice fed high-carbohydrate diets (PMID 37030441).

Has FGF21 been studied outside metabolism?

Yes. Cardiovascular work reported that FGF21 protected against atrial remodelling by reducing oxidative stress (PMID 34708083) and that it inhibited inflammation and fibrosis after myocardial infarction via EGR1 (PMID 34478691). In joint tissue, the study reported that FGF21 alleviated senescence, apoptosis and extracellular matrix degradation via SIRT1-mTOR signalling in osteoarthritis models (PMID 34556628).

Why is FGF21 measured in blood tests in research settings?

Circulating FGF21 concentrations change with metabolic and organ stress, so clinical studies use it as a readout. One 2025 analysis evaluated circulating FGF21 as a prognostic and diagnostic biomarker in hepatocellular carcinoma (PMID 41399394), while another reported that FGF21 predicted and promoted vascular calcification in haemodialysis patients (PMID 34179118).

Do any studies report unfavourable FGF21 findings?

Yes. Researchers reported that FGF21 predicted and promoted vascular calcification in haemodialysis patients, an association that is not protective (PMID 34179118). Elevated circulating FGF21 has also been examined as a disease-associated marker in hepatocellular carcinoma (PMID 41399394). Observational measurements cannot establish whether raised FGF21 is a cause, a consequence or a compensatory response.

How strong is the human evidence for FGF21?

Most mechanistic findings summarised here come from rodent or cell models, including brown adipocyte, cardiac and cartilage work. Human data in this reference set are physiological or observational, such as a study of FGF21 and fructose dynamics in humans (PMID 30338119).
